In terms of having the game constrict you I completely understand your position, so here are some suggestions... they may or may not work, but I hope they do.
During a rather quiet period of my game I used Custom Battles (thanks to Radzeer to giving the clues about this) to provide some much needed action, and then simply leading the plot where you want it to go. The last 7 chapters in my AAR are all through Custom Battles and nothing else. If you need a character to die then send him off to a un-winnable battle, save the pop-up and then post that - your readers don't need to know how he actually died, you can make up some heroic death , or whatever death suits the character.
If a character dies that you didn't want to you can simply not tell us and play on... we won't know will we?
For example in my chapter when Eustace talks to King Stephen in-game Stephen was already dead!
Although I do always find it quite fun to accept the death an play it out, if you weren't expecting the death surely your audience won't? What more of a twist do you need.
I hope this babbling helps in some way or another.
Oh and about your second paragraph, it is only now that I've started planning it in the future, before that I just went as it took me.
