Perhaps. Two things:
1) Are you audio drivers up-to-date?
2) Switch to the Miles 2D positional audio or just to a different audio setting. Your current one may not be supported by your hardware.

I got the same problem. I downloaded and installed Rome Total War, patches 1.3, 1.5, Rome Total Realism Platinum Edition, and Rome Total Realism Platinum Edition Add-Ons (in that exact order, with precision and grace as the official website said to) and as i tried to run Rome Total Realism Platinum Edition it said "failed to initialise 3d audio". Rome Total War works fine, the patches installed, but for RTR will not operate because it "failed to initialise 3d audio". I have MSS32.DLL in the RTR folder so that can't be the problem...
I'll try copying MSS32.DLL to RTW files to see if it works then, because I took MSS32.DLL out of RTW's folder...
In the mean time...

Take note, I have not previously used RTW, these were first time downloads and installations, so I didn't cross any wires...that I know of...

Okay, I copied and pasted the miles folder from RTW to RTR, this time I didn't get the "failed to initialise 3d audio message", RTR opened and a map of the regions of italy appeared, it was the pre-loading screen stating all the copywrite stuff and that ****, but then RTR closed abruptly and returned me to my desktop with no error message. I re-tried multiple times, but only to have the same thing happen.
HELP!!!

What version of RTRPE do you have? The simple version or the mod switch one? What do you mean by Rome Total Realism Platinum Edition Add-Ons? That is a collection of add-ons. What have you installed from it? Or did you mean the RTR 6.2 Music add-on?
My guess is that somehow you`ve messed your install and by this point you`d probably be better with a complete game reinstall(uninstall and delete the game folder after that). If you do take this step make a backup of the game folder before installing the mod next time. It will be easier to revert any changes you do to your game.
I am using the normal version of RTRPE, not the mod switch version. Yes, the rome total war platinum edition add ons are just as you said, a collection of add-ons. i installed every single one (as directed by the website I got the material from, here is the link, i downloaded the platinum version and followed all directions to the letter) no, i do not mean the RTR 6.2 music add-on, i do not know what that is.
i will try, but i do not know how to unistall it, in what order, and all that BS. make a backup of what game folder? cmon, bruh, you gotta be more specific. go to the link up there and do what i did, see if you can get it to work. unless you can be more specific or provide any more suggestions, i do not know what to do.

The music add-on is a bit lower on the page. It adds the RTR music instead of the vanilla RTW one. If you want to install it just dl and follow the instructions. It`s pretty simple as it has its own installer.
The add-ons you`ve installed are probably compatible with one another, but I haven`t checked it for all. You should read the readme file included in each one. It should say.
My advice was to :
1. uninstall the game
2. delete the game folder
3. reinstall the game
4. start the game. See if it works ok
5. go to the Options menu in game and look at the upper right corner. It should say ver1.5
(if it says another version then post here again; you`d have to upgrade your game before installing the mod)
6. make a backup of the game folder. This means that you copy the whole game folder(Rome - Total War) and place the copy somewhere else on your drive. If you mess the installation after that you can simple delete the modified folder and replace it with a copy of you backup. Better then to reinstall
6. next, if it did say ver1.5 in the Options menu, install the mod
7. start the game and see if it works before installing any other add-ons.
I got it working!!! Woo! Man, RTR is so much cooler than the orig.

I would guess it's because of installation of the wrong folder. This sort of stuff comes out during the early phase of conversion I think (back in old rtr forum).
