Actually, I've had captains not in the first roster place be placed in charge. This only happened to me if i had an elite unit that wasn't in the first roster position, and i believe if there are more than one, then the one with more experience or higher recruitment cost is picked to be in charge. But in an army full of levies, it just defaults to the unit in the first roster position.

Heavy cavalry will usually be picked instead of the first unit if the first unit is infantry.
