Well obviously any faction you are at war with won't vote for your Preferati to become Pope, but what about neutral factions? Does the game base their decisions on reputation or power etc? Or is it just completely random??

If I remember correctly, factions vote for their own first, then their allies, then the one who has the most piety, and finally the one with the most eligibility. You can also buy the votes.
How do you buy them? All this time, Popes have hated me when I could be paying them?

I thought the standing with the faction and the reputation of your faction would matter as well for neutral factions.
Simply enter in the diplomacy scroll by clicking on the arms of the faction undecided, in the voting list.
If one of your cardinals is a candidate in the papal election, at the bottom of the scroll where you place your votes you should see logos of different factions.
These are factions that will be voting in the election. Clicking the logo of a faction will open up the diplomacy screen, and you can "Demand papal vote" or something similar. It usually costs a lot of money but it can be worth it.
