I may have missed something in the rules, but what is the area your allowed lay claim to when you create a character? For example, could I be Duke of Milan straight off or would I have to work up from Viscount of Lombardia itself?
Do you still receive the standard 5 items of choosing on start up?

As cho'gall says, you have to start of as a viscount. But as the city of Milan is in the region of Lombardia you can name it the Viscountcy of Milan. I mean, ForteS is Count of Toscana yet there is no region named Toscana.
What does province Tier refer to? Specifically I'm trying to calculate Levies.

What up, Im a GSTK Grognard(or more accurately guard polish lancer) and I was wondering what if any is the difference in the rules between this and GSTK and is the postion of the Duke of Silesia open, perhaps as a Piast? And what is the game rule and who close are we playin it to history
Also, is the Margraviate of Meissen or perhaps Brandenburg or some other eastern/baltic border areas open
And are we having the baltic and oborite crusades

The Duchy of Silesia is a part of Poland currently. We restarted in 1000 AD so the empire is somewhat smaller, but still powerful. Boleslaw I, who is a part of the Piast Dynasty, currently rules in Poland. So you can say you are distantly related to the Piast Dynasty, but all the territory in Eastern regions of the HRE either belong to the Duchy of Saxony (Cho'gall), Duchy of Bohemia (Me) or Duchy of Austria (Stig). Unless you wanna be a vassal of one of those three duchies, there is only one territory that you could try to gain as a count from the Emperor, but would be effectively surrounded by the Duchy of Saxony and Bohemia.
Besides, you can't start off as a Duke right away. You will most likely start off as a Baron and by capturing other counties, can be elevated to Count and/or Duke, but that is not guaranteed as M has stated many times. Most people had their stuff transferred over, but right now, unless you wanna have a Duchy in the long run, I would recommend starting in Italy or in Western parts of the HRE. If you wanna start in the East, you can be a vassal of Bohemia, Saxony or Austria.

Hmmm could I rock a religious mercenary dude and we start some kind crusade thing against the pagans,maybe start the north crusades a bit early, I would be cool with running an organization like the brothers of the sword or something
and I was thinking of poland in the 1100s when the Piasts came apart

Well currently I have plans to make some diplomatic ties with Poland through a marriage alliance. And then go from there. A crusade would be interesting in the Eastern parts. I believe only the Lithuanian area is pagan as Kievan Rus is Christian under the reign of Vladimir the Great who married Anna, sister of Emperor Basil II of the Byzantine Empire.
Hmmmm...well I gave you your options, just choose what you want.
- Vassal of Bohemia, Austria or Saxony
- Build your own base of power in the western provinces of the HRE
- Your idea lol
