Well in my very very first M2TW campaign, first time ever played TW game, I started as Spain, I built only army, went bankrupt, I was at multiple wars, so the only solution for me was capturing and sacking Rome! Not the best idea, really. So I went bankrupt again and........I gave up. It would be defeat.
This only ever happened in Shogun original and Medieval TW. Actually I recently played a MTWXL campaign as Norway and I got my ass handed to me. The AI gets a significant power bonus due to the provincial paper map. On the 3d map it just can't compete... campaign AI is utterly stupid.

Never had 2 generals rebel at the start before, that is very unlucky.
Parthia's infantry is terrible, but there cavalry do make up for it though, just about. Problem is, you can't recruit any of there decent cavalry till later on, if you get that far that is.

I also lost in RTW BI as Sarmatians. I tried to survive Hunic invasion without hording and suddenly lost my last FM due to assassination. Second time it happened to me in Arthurian Total War (RTW mod) playing as Elmet but this campaign is extrremly difficult because of scripted Angles invasions.
If you haven't lost yet. download Stainless Steel, and choose VH/VH with BGR and Gracul AI. Hardest campaign I've ever played.
Not been beat on either Rome or Medieval, but I've been close on two occasions on Medieval. The first time was as the Turks on VH difficulty on vanilla. Mongols came for me, I got hammered, and became a vassal of the Mongols. The second time was as Venice, because the HRE makes so many assassins, and I was down to about 2 family members, with my FL being 58.

No defeats for me; Milan once came close when I was new to the game and just rushing for the victory conditions though. Half out of boredom and half out of difficulty I quit that campaign for a month came back and realized it wasn't quite so impossible (Basically using the stuff I had learned I came back from the grave)... I achieved world domination in that campaign.

I have only ever lost a campaign in any total war game twice - both times, strangely enough, as the Apacheans...
The first time I just wasn´t fast enough, and some other faction won first.
The second time, however, was as OP describes - My faction got completely annihilated. This was due to the Apacheans starting out with so few Family Members (and usually adopts a few more early on if you expand fast enough).
Within the first 15 turns, two of them rebelled - which caused two of my conquering armies to disappear - and a third was assassinated, leaving my 60-year old faction leader as the only Family Member left.
The next turn, he died of old age...
So it was bad luck, really, and not ineptitude in leadership, which I suppose is at least kind of comforting.
