Your reputation doesn't t take a hit but your relations do. Relations can fall and a faction may choose to declare war if you repeatedly spy. If you use assassins on family members, your relations fall. Also when attempting to assassinate a family member, if you fail and the assassin is caught the faction will declare war after interrogating him.
Another problem with spies and assassins is that you need to build the inn/brothel lines of buildings. Get ready for the "Drunken", "Gambler", "Womaniser" etc. type traits for the Governors of such settlements and any General/Royals that happen to stay in a settlement with such buildings.
Hence why construction of the buildings for spy/assassin training generally needs to relegated to cities that you feel won't be your primary economic powerhouses (and hence likely to have a governor) and NEVER at the Capital or the cities where you educate your characters.
Also note that use of assassins will add dread to your faction leader fairly quickly which can be an issue if you've been promoting chivalry for the potential bonuses especially the population growth bonus. Having a decently chivalrous heir move into the faction leader role while maintaining an active group of assassins tends to result in everything cancelling out after a bit which can make it hard to establish certain good traits in successive generations of family members.
So...you order a hit but can't take a hit?
