Release prisoners and occupy settlements. Do what you promise, too.

And if you're at war making an alliance is much more difficult (especially if your enemy have allies).

Keep giving 1k florins over and over to get relations to perfect. Then, make offer of alliance.

I don't know if it is a bug but...

Today, I tried to make an alliance with Hungary (I'm already allied with Poland, that is allied to them). So, I kept trying to make an alliance offering various kinds of payments and regular tributes. I was being generous, and they kept refusing. However, as I kept giving them more offers my relations to them were increasing. In two turns of giving several offers our relations reached Outstanding, and then I made a ridiculously generous offer and they allied with me (relations Perfect now).

That seems normal. I don't see what aspect of it is suspicous so as to be a potential bug.

I know that, the more alliances you have, the easier they are to get. Then the obvious, releasing prisoner, occupying settlements, and not starting wars with everyone.

Every time you make a generous offer to a faction, your relations improve, regardless of whether or not they accept.

personally I make alliances with everyone at the beginning of the game (except my first target) since it will make relationships increase early on. then the alliances begin to filter out anway through war AI vs AI or war AI vs me

One aspect that I dislike is that neutral armies can march all over your territory and attack you at will.
If you try to do the same, your diplomatic standing suffers immediately.

If you repeatedly make generous and very generous offers you status with that faction improves. The easisest way is to check their financial status (eg meagre, bankrupt) and offer to sell them several provinces for 8000 florins. they will just refuse but your relations improve. The down side is that each time your diplomat fails to make a deal his ability is decreased so if he keeps offering this type of deal he won't be much good for anything afterwards.
