I think in Stainless Steel you can do it, I am not sure since it has been a while since I have played.

can't be done without modding traits and ancilleries to give massive amounts of authority, and even then you must plan your heir out while your next faction leader is still heir himself. there are some mods out there that allow you to move authority granting ancilleries and traits between generals/family members to try to plan out your future heir. i find it rather annoying. more info can be found HERE.

Zealot's right. It's up to your authority, Stainless Steel doesn't allow you to choose your heir.
