Milan was excommunicated, yet it's popularity in Genoa(I took Milan) was enormous. So what are the real effects of excommunication? Will other Christian nations really attack you? Will you really lose your people's support?

I know the happiness of all the provinces goes down by a point or 2... Relations with the pope will be strained.
Your more likely to get a informative explanation by using the search engine.
You will probably also get a crusade called on you (My experience)
I was Poland and I got excommunicated for sending an attack on England and Scotland. But no one came so...
30% PO penalty in cities(from experience I would guess 15% in castles)
You can't join crusades, and if you have joined one you abandon it
A crusade can be called upon your provinces
Other catholic nations will be asked to break alliances with you by the pope, and probability of war with catholic factions is increased.
If you can choose when you get excommed, wait till a crusade is called, or in progress so one can't be called upon you.
It’s better to excite some and offend others than be bland and acceptable to all

Just stay in Popey's good books and use him against your enemies.
in my campaign of the HRE, i wasnt excommunicated and the pope launched a crusade against vilnius, that is mine. i think i the crusade was launched due to more than 30% of the population was heretic, i had two heretics in that province
Not being able to join crusades is a real disadvantage, it is imperative to use the free troops and movement bonus whenever you get a chance. Other unhappy results of excommunication are listed above by crzyrndm.
PS: NEVER get excommed while on crusade, you will lose the entire army except for the generals.

Also, your priest might turn heretic, and it will increase the number of inquisitor on your land. It might also lower your king's piety or give him bad traits, I'm not sure since I don't micromanage too much. The negative effect on public order seems to grow over time.
Crusades WILL be called on other Christian factions. France got excommunicated in my Venice campaign, and the Pope called a crusade on Toulouse - which got taken over by Denmark. France then became a vassal of Denmark lol certainly the strangest occurance in any of my campaigns on MTW2
Cry God for Harry, England and Saint George!
A crusade will almost always be called on Toulouse if the owning faction is excommunicated. It's coded that way to reflect the historical Cathar crusades against Toulouse. If you don't own Toulouse, you have less to worry about.
hmmm. strange. i've been excomunicated plenty of times while owning tolouse and there was no crusade called against it.
in fact the pope never seems to call for a crusade against excommunicated factions unless i ask him to.
Maybe he only calls on crusades on excumm'd christians if it is the player side. The Pope has a certain groen in hatred for players...
I've had Crusades called on me when excommunicated, and I have not had Crusades called.
If your faction leader or the Pope die, you will usually get reinstated.
"Every idea is an incitement. It offers itself for belief and if believed it is acted on unless some other belief outweighs it or some failure of energy stifles the movement at its birth. The only difference between the expression of an opinion and an incitement in the narrower sense is the speaker's enthusiasm for the result. Eloquence may set fire to reason." -Oliver Wendell Holmes Jr.
I have been excommunicated countless times in 3 of my campaigns, but only had one crusade called on me ever. It was when I was Venice in Northern Africa (forgot the name). It is actually the only time I've had any religious war called on me, and I'm playing as Byzantines now at about turn 80 and no Jihad on Constantinople, strange.
Considering it's Milan, absolutely nothing different will happen than if they stayed in the church.
Jon had taken Donal and Benjen’s advice to heart: Sam may be fat and pathetic, but he is still a member of the watch, and one of the few black brothers who isn't a rapist or thief. (out of context, this sounds ridiculously racist)

I like playing with Catholic nations only because that the Pope is the only higher authority than the king of my faction in the world XD
No, I like it because when he tells me to stop fighting a nation when they're down to one city, it gives them a chance to regroup... More of a challenge.
