how ican open medieval tw folder like the text file and others same as rome tw

You need to unpack the game's files. In the game's folder you find a tools folder, in there you find the folder for the unpacker. In the unpacker's folder you find some batch files for unpacking things, and a readme.txt. Read the readme.txt, it will explain how to use the unpacker, and how to run the game with modified files.

Edit: Also, when the guy talks about the choice between mod folder and io.file_first, you should probably go with io.file_first since you're used to RTW. It's basically the same as how it was in RTW. Mod folder is a little bit different.
