How do I make unit sizes HEAPS bigger?
Well I know you can do this in Empire and Napoleon by just changing some numbers in the preference file, but is it possible to change the unit sizes in Medieval to sizes like 500? Or are they set and unchangeable?

Yes, it can be done, but 251 is max 
you can edit it in export_descr_units.txt, but then you have to do it for each unit.
Then you have to edit the base size (marked in red) to a maximum of 100, like here:
type Italian MAA
dictionary Italian_MAA ; Men At Arms
category cavalry
class heavy
voice_type Heavy
banner faction main_cavalry
banner holy crusade_cavalry
soldier Italian_MAA, 32, 0, 1
mount barded horse

Note: You should expect more problems in siege AI due to having units that will not fit on a wall section which can be attacked with ladders or towers. At times the entire AI can freeze as a result. The possibility of this occurring can be reduced, not eliminated, by tuning the BAI.
Check my work or Germanicu5's for good examples of BAI designed with large units in mind.
