Personally, I use this setup:
- My sub-mod Compilation installed
Sub-mods:
- Savage AI
- Istanople
- Live Long and Prosper
- Permanent Arrows
- Historical Regions and Settlement Names
Settings:
- Early 2TPY Campaign (included in my Compilation)
- Very Hard/Very Hard
- Usually Norway or the Romans
Note: if you're new to SS and/or Medieval 2, I recommend Medium or Easy difficulty. And don't enable Byg's Grim Reality unless you want the game to be much harder and complex.
Savage AI, options Latin Empire submod for late campaign, VH/VH. The good is like byzantines things are challanging and interesting and have the feeling things are close to perfect. Suggestion, play what you like, factions you love, trust your ways. Recommandations, try to play early, keep the RR way of game, play agresive. My factions are Teutons, HRE, Romans, France and Turks. All have a lot of fun.

I usally play on H/H. Favoured factions are Poland, Novgorod and Sicily.
I play with MSC plus BGR, Longer Assimiliation and Limited Actions on M/M, and enjoying every single turn.
And my favorite faction is Castille.
I use Meneth's compilation and Gracul's aggressive AI (I do not use BGR). I usually play the Byzantines (Romans) on VH/VH. I like this set-up because I like fast and furious action -- shifting war from west to east and back again.
I think the most difficult thing about playing on VH/VH is managing money. I've learned some ways of doing that. Perhaps I'll share them in another post some time.

No all out favorite factions- Genoa, Lithuania, and CS played the most often. Longest campaigns were Portugal and Turks so go figure.
Most boring campaigns- England, Venice, Byzantines, Fatimids.
Most intense but also tedious- HRE, K-Shah, Hungary.
Factions not played yet- Kievan Rus, France, Moors, Castile.

Does Real Recruitment give me less or more troops? Might be a silly question but I'm just not sure.
Disabling it is broken.

RR gives you less troops, yes, but the AI is bound by the same restriction.
