what do you really need for chapter guilds are governor with a minimal 5 chivalry sitting on a certain settlements; joining crusades helps too and perhaps a more quicker way; but I find joining crusades with multiple armies are simply not worth it, especially when I'm at war with catholic neighbours and the crusade distance is far away.
Did you have any CTDs? I've had two or three campaigns in which the guilds were screwed up, and I suspect the CTDs I had at close to the beginning of the campaign were to blame.
Playing as Aragon, I only got a minor Montesa house in Toledo, and nothing anywhere else. I left high-Chivalry generals in there for over 100 turns, and recruited lots of Montesa knights, and I never got an upgrade offer. Playing as Norway, I took over Arhus, destroyed the Theologians' guild in it, and never got a Merchants' guild offer, even though I upgraded the city, built the top two markets, cranked out about 20 merchants one after the other and maintained a high faction income. I have several other guildless cities which normally should have gotten the Merchants' guild offered.
silly question, but they were large cities and fortresses right? that's when they are allowed, then huge cities and citadels allow the other 2 upgrades

yeah some people tend to forget settlement level requirements
most of my towns are fortress/large cities, but the lowest level teutonic knight chapter house become available at minor city and fortress level, still dont have a single one offered to me yet.
For knight orders you need a minor city for a minor house, a large city can have a major one and the HQ. Fortresses can have a minor house, and citadels can have the other two. Yes, Arhus is a huge city, and still no Merchants' guild, and with Aragon, all my castles were citadels.
Fastest way is to Crusade, but the trigger is based off the number of times you have an army 'join' a Crusade. Need 100 points toward the guild, minimum, for the offer, and each turn it'll trend toward zero, so you have to keep working it. You get 25 points for each region for each general that joins a Crusade (you also lose 25 points for all regions per general that leaves the crusade). So, Crusade with whatever you were planning to, and then just before you capture the target settlement have as many other Generals join as possible (note there's a minimum army size accompanying the General). So, if you can join with 5 Generals you get 125 points, which should keep you above the 100 point threshold long enough to get an offer somewhere (assuming you have cities/castles of the appropriate size that don't already have another guild built in them).
For the HRE, in particular, you get 10 points per turn for any region you own that is adjacent to a region with >50% pagan religion. So, get that settlement to minor city or better so it can be eligible for a guild, and don't convert neighboring regions that meet this pagan requirement until at you get the guild you want built.
The 5 or better Chiv general parked in the settlement will help for certain guilds, namely Santiago, St Johns, Montessa, Templars, Teutonic Knights, and Woodmens (English only). You get 5 points per turn towards these guilds (assuming they're available to your faction) simply by parking the general in that settlement.
This is the hard part, getting the lvl 1 guild to lvl 2 and then lvl 3 is actually MUCH easier, as you get points for any recruiting of units that the guild offers, which was the whole point of getting the guild in the first place so its not like you won't be doing this.
Santiago and templars are the tougher ones to get, the only triggers are for Crusading and governor of 5 or better Chiv (and recruiting the units in the guild once you have it).
The other chapter houses have additional triggers: being adjacent to a >50% pagan (teutonic) or muslim province (montessa, st john), or declaring war on muslims (montessa, st john).
