If you have too many generals in proportion to the cities you have, you won't get new births. If you want kids, don't accept adoptions, and/or take more cities.
Use the worst ones as "tanks" in your expansion - either they die freeing up the slots for children or they get better and create slots by conquering new land... of course using them alongside already good generals works perfectly fine as well.
There's also a limit of 4 children per couple. But you can change that in an XML document somewhere.
