    Uhh, that's not really the end exactly...

    In case you couldn't tell the story could have gone either way the entire time. He was either sane the entire time and they tricked/brainwashed/drugged him, or he was actually insane, you don't really know. All you do know is that he was completely and utterly sane by the time he made the decision to have that brain operation done, because of the last line he said. Also the doctor said his "fake" name to him and he didn't respond. When you go through the entire movie, it makes sense either way with the entire thing. Personally I think he was actually sane the entire time, and they drugged him and brainwashed him at the end, but that goes against what most people think.
