    Default Ninjas!

Well not quite, but close. Over the course of 3 turns, I have lost 5 family members in Northern Europe (as Bretons) to assassins I cannot see. Normally (at least in the last two campaigns I've done) assassins show up on the map as any normal unit, and usually fail. But FIVE family members in one go?

    Spies!!
Assassins should find it very hard to kebab your generals if you have even just the one spy attached to them.

Spies are a truly indispensible tool of warfare, especially in highly contensted areas where you have to deal with high unrest.

Definitely. A spy in a city is a great way to keep other spies from causing unrest, and they really do wonders to keep your generals alive.

Yes they do, I thought everyone knew that. They also cause a lot of unrest. One could theoretically destroy an entire empire with an army of spies without marching an army.

Yep. Quite an effective way at causing havoc in a factions peripheral settlements, especially with a poor (or absent) governor. Of course, this is also why you need spies yourself as part of a settlements garrison.
