All technical changes for the most part, directional changes, adjustments to core documents ie, modding policies/terms of service (a big factor to the slowdown sometime back in resolving issues with modding when that was a big thing in the QS). Besides that a bottleneck where as Pike noted, a good portion of the administration is also inert, leaving us firing on roughly half cylinders with a low amount of candidates for Hex and limited agency even in becoming one as 'status quo' iirc was the directive before Ged became almost entirely absent.
After that it falls to volunteer motivation. There's an unfortunate cycle here where there are few who are willing and carry trust while there are a few beyond that who are willing but I think it's fair to say, don't make themselves the most desirable candidates. So even if it's possible to fill some places on paper, the odds have to be weighed between leaving a certain thing inert or risk it dropping even lower. Otherwise as evidenced by the many holes in the staff list there is straight up a lack of interested people, leaving functionaries like Hex spending their limited time covering gaps. So while certain actions may not require his approval they do require parties who can simply upfront the benefits, and also carry out the work. For me, I have a number of aspirations both on the wiki and in forum sections - cleanup, streamlining, cutting purely obsolete content or updating it. The sense of emptiness and lack of backup I freely admit holds me back from putting much time in and I've found myself much more interested in pursuing fresh projects. The entire apparatus slows accordingly.
Noting my limited access (I'm certainly not a Hex and this is generally based on public notes and inputs) and that I can only speak for myself, these are key contributors to what is, yes, not an ideal status quo. The absence of the top shot caller regardless of direct approval or not is very demoralizing to a system and a site that's been on a downward spin. If initiative cannot come from the top down then you have a very large hill to climb to try and bring it from the bottom. I'm sure MCM could corroborate, ex-staff from the old days who came with a fresh look and a lot of initiative who eventually surrendered to the very difficult circumstances.
