Well its mostly for Role play purpose. If you dont care about keeping the royal bloodline in tact then adopt away but alot of people(including myself) like to keep the bloodline pure. Also there is the traits your family members who are descendents from the starting faction leader such as " of (insert faction name) blood, usualy giving some sort of small bonus.

for example, intelligence and looks are the most common hereditary traits. i did a little tweaking in the descr_strat file, and nearly all my generals are good looking and intelligent.
Also, its cool to keep a pure blood line, for more loyalty and etc...
And it reflects real life, as all medieval states took great care to get the right man on the throne, most often the eldest son of the King.
In the campaign im doing now, im not letting the women in the family marry, so as to keep the original family name. Just because it looks cool.
Theres something I dont get though... Obviously its tradition that the woman takes the mans name, but wouldnt you think they would disgard that rule when the woman is royal? Pretty strange if you ask me.
It cant be cruel, it is a game and he obviously wants a traditional medieval (ideal for the time) blood line instead of the random marry into the familly dude getting the faction leader spot and then him having all daughters which the game likes to do. It annoys me as well.

Not just for roleplaying. Each faction has a traitline for "Royal_Blood_<factionname>", some of which are quite good. For instance, Hungary and Poland get a 10% movement point increase for family members that are of royal blood (ie. direct male descendents). Russia gets a +25% movement point increase. Other factions often have a +/- Chivalry trait, some get a morale boost, others +law or +trading.
Many players, including myself, NEVER accept adoptions or Man of the Hour, for a variety of reasons (be nice to have a game option to disable them). When playing a faction that has a good Royal Blood trait line, it can also be worthwhile, for the pragmatic reason of getting a nice trait at birth on your family members, to try to get as many direct male descendents as possible. You can do this by declining to marry princesses (except to give them in marriage to AI factionleaders/heirs) and not allowing any male family member to marry if he is not a direct male descendent (ie. if he does NOT have the Royal Blood trait line, he can't pass it on to his sons). Once you start doing this, its pretty easy and, oddly, has no effect on the number of generals you have, since the game varies your birthrate based on your current number of generals / #provinces owned.
Or moddifying the game so that princesses can pass their royal blood line to their children.
