I've personally made sure my armies have slaughtered all of their generals and family members, yet somehow they still survive with no settlements and no named characters besides a spy and a diplomat.

same im my rohan campaign m sure ive taken all their settlements but havnt been destroyed
there is a fix for it, I guess its in the PATCH 1.2 RELEASED topic...

Be warned however:
By using this Fix, you may find that your gameplay is diminished if you kill the leader of the following factions:
Mordor
Isengard
Orcs of the Misty Mountains
By killing their leader, I have found that their entire faction is destroyed.
Nothing sucks more than being proud that you have killed the Witch-King in battle, only to then realize that you won't even get to conquer Mordor from hordes of orcs anymore...because all of Mordor is now garrisoned by Town Militias of the Bandit variety.
EDIT: I should note that I have reinstalled my game and hope I have wiped out this fix so that I can go back to NOT wiping out whole factions by only killing one character.
It is now much more fun hunting down the last remnants of a faction and destroying the "rebels" with my army and agents.
