Theoden was not crushed by a Mumak. The Witch-King brought down his horse with a poisoned dart, and the horse crushed Theoden as it writhed in pain.
Also, you forgot Boromir's death.
You should have included the great heroic death scenes from the Silmarillion

. These include:
--Feanor's last stand, when he died surrounded by orcs and his fiery spirit consumed his body after he expired.
--Huor's death at the Nirnaeth Arnoediad, fighting at the side of his brother Hurin, who was captured
--Turin's suicide, when he threw himself on his sword Anglachel
--Beren's death at the claws of Charcharoth
--Glorfindel, who fought Gothmog and each killed the other
--Fingolfin's death as he fought Morgoth one-on-one
--Fingon's death at the Nirnaeth Arnoediad
And many more...
