Edit: so while I was writing this post asking for help I figured it out, so I'm going to post it here anyway with the solution hoping it helps somebody else
I tried to search for this problem, but had a hard time finding the right search terms to use. I found this thread that describes part of my problem:

I have MTW2 Gold, running on an ATI 4870, 1920x1200 widescreen LCD. When running MTW2 non-Kingdoms, on the campaign map the graphics look stretched horizontally or squished vertically, depending on how you describe it. Units look short and squat. I know that the UI elements are "supposed to" look that way because they didn't create a widescreen version of the UI, but the actual 3D map isn't supposed to look that way is it? Kingdoms looks ok, but MTW2 vanilla doesn't.
Even worse than that is mouse clicks in MTW2 are offset from where they should be unless the item I'm trying to click on is dead center on my screen. For instance if a city is on the right side of the screen I can't click on it if I hover directly over it. I have to be just to the left of it. If a city is on the left side of the screen I have to hover just to the right of it. It's like the game thinks the screen is narrower than it really is. When this really becomes a problem is trying to direct a unit to go to a spot on the map--I always miss because where I clicked isn't where the game registers my click. Again Kingdoms works fine, MTW2 vanilla doesn't.
I think these two issues of squished graphics and inaccurate mouse position might be related. Anybody encounter a similar problem and know of a fix? Thanks ahead of time. This problem makes the game pretty much unplayable for me.
Edit: found solution. I have 2 monitors. Primary is 1920x1200, secondary is 1600x1200. Main is actually labeled as monitor #2 while secondary is monitor #1 in display settings. When I have both on and 1920x1200 one as primary is when I have the squished graphics and mouse click weirdness problems. So I turned off my 1600x1200 monitor and that fixed the problem! Looks like MTW2 vanilla gets confused about what monitor is actually running the game, and turning off the 4:3 monitor fixed the problem. Hope somebody else benefits from my hours of head-scratching.
