For battles you should use H or VH
for campaign it depends whether you like to explore more diplomatic approach. As the difficulty goes down, you get more opportunity to make diplomatic alliances even with historical enemies.
Typically H/H - regular
VH/H - some challenge in battles
VH/VH - challenging campaign overall

I've been playing on M/M and while the campaign difficulty seems reasonable, the battles are certainly too easy. I'll give M/H a try.
im playing with KoJ in VH/H and i have a lot of fun the eggyptians atack me every turn and i have to take a strong care of economy or i dont have an army to defend jerusalem i was forced to make a fleet to reppell the abuyyds navy they were suffucating my economy( koJ depends of the ports to hv some income) it has been hard but fun. now i find the best defense is the attack i siege gaza with Balldwin and heroic win 2 stacks and take the castle, im sieging with a small force the city of damascus, i belive as some income but it will be a very hard battle.
For me the best combination is H for the campaign & VH for the battles
I like to have some opportunity for diplomacy etc which seems absent inVH
Also in VH campaigns the other factions seem to have huge resources which were impossible when I play as them!
Can someone tell me if the bloody Mongols actually do anything on the higher campaign difficulties? On Medium they seem to just stand around most of the time, it's 1227 in my current campaign and they have not taken 1 settlement yet.

Actually, in the past four campaigns of mine (ERE, Rajput, KoJ, Abbasid), the Mongols army only is perhaps half the size of the Khwarezmian's full force. As such, the Mongol event is hardly a game-changer. It's become a constant like the KoJ always losig early to Ayyubids and Armenia not expanding.
On topic, though, I find H/VH nice and challenging, if nothing else but to see my elite armies crush numerous full stacks of levy units the AI thought could beat me.

Funny you should say this, in my Makuria game Armenia was the #1 faction. Not so constant.
Also, half the size of Kwarezm's force should still make the Mongols a game-changer because the Shah is usually very spread out by that point, and Mongols emerge near the lightly protected rump of their empire with potential to take a number of good cities in the first few turns. But they squander the opportunity by acting so passive.
