First of all, squalor (I'm assuming that's what you're talking about) is caused by population, methinks. If your settlement needs to be upgraded from a town to large town, large town to city, etc., do that, as it will greatly improve public order.

Recruit bigger garrisons as well. For cities inside the heart of your empire that are not threatened by attack, recruit units of peasants. I don't know what unit size you play on, but on huge, recruiting peasants does a lot - it reduces the population by 240 and increases the garrison bonus.

Some say not to build Sewers and Land Clearance (and anything above those), because population growth rises if you do, leading to more squalor, which makes them more harm than they're worth. I play on Medium campaign difficulty, and build public health buildings but never farms, and that seems to work fine for me, but if you really want to keep population growth (and therefore squalor) down, do not build public health buildings.

Population growth causes squalor. Nothing you can do to get rid of it, you can only try and counteract its PO penalty with temples and health buildings.
