A city dropping below 75% happiness for about two or three turns. The whole city revolts against you.
    Captains can desert you
Rebels appear if you've got too much law in a city or region.
thats as much as i know, hope it helps

I know loyalty of your Generals begin to drop, but I've noticed when I have a Generaless army outside of a settlement and my country is doing bad, they disappear.

That's happened to me too, I once built a massive army to crusade for Vilnius, my good generals was off in britain so I took a 0 loyalty 4 command general, joined the crusade, and then 1 turn after I'd joined the crusade the general just left and the army deserted the next turn.

    You accept 0 loyalty generals?

I decline any generals who have the "mixed loyalties" traits or worse, and I either use family members with low loyalty for city minders, or get them killed.

Happiness doesn't necessarily have to do with rebels randomly spawning in your country-side. I have rebels quite constantly spawn in my constantinople region that has around 240% happiness.

If you have enemy soldiers in your region, rebels seem to spawn quicker. The thing is, if they do appear, get rid of them ASAP! they cause devastation to your land, ruining your farming income, and cutting off your trade routes
