As far as I know, you can re-texture them.
In LOTR the film, castles are very cool, but i think it could't be made in medieval2, unless it's not all same with the film.

I don't really think you can make Custom Cities, well,.. Not yet anyway.
Some people have managed to suss out the Codes and I've seen some pretty impressive tricks in some of the Mod Topics.
But as for making it in ways of doing Custom Cities like Helms Deep or Osgilliath?
I think is a long way away and hopes of it being introduced to the Third Age:TW is highly unlikely.![]()
