Your armies are suffering from attrition.

When you select that army below the total number of troops there is a percentage. That is how many of your troops die off per month and is affected by supply limit of the territory your in. If you're in your own territory, you only get attrition if you have more troops than the region can provide for but if you are in enemy territory then the region will inflict atleast a small amount of attrition anyway unless your army is really tiny.

It is caused by no supplies. every time you check a region, it shows you the amount of supplies. you have 1 supply for each regiment(or for each 1000 soldiers, I'm not sure), if you don't have enough supplies for your army, they will begin dying.

It's called attrition, and it represents the casualties of disease and bad supply. When you click on a province you'll see that each one has it's own supply limit, which is the maximum number of regiments that can be located there without suffering attrition.
The attrition % of the army can be seen when you select one.

    But it's reality.

So plan ahead and you wouldn't have this problem.

In before the "reality is stupid, I hate this game"
    inevitable post.

IMO its fine. It's just yet another feature that makes this game far more interesting, realistic or not it makes it deeper and less arcadey.

In contrast to the efforts of tiny Israel to make contributions to the world so as to better mankind, one has to ask what have those who have strived to eliminate Israel from the face of the earth done other than to create hate and bloodshed.

I just act the merciless tyrant and ignore it - who cares if a few hundred peasant soldiers die? So long as there are plenty to spare...

The only place I've really found it to be significant in my planning is on the steppes - my Ming invasion of the Oirat Horde lands didn't go very well - support level 1 provinces and 10,000 strong armies don't mix very well . I like it though - the steppe should be hard to invade.
