Your armies are suffering from attrition.
When you select that army below the total number of troops there is a percentage. That is how many of your troops die off per month and is affected by supply limit of the territory your in. If you're in your own territory, you only get attrition if you have more troops than the region can provide for but if you are in enemy territory then the region will inflict atleast a small amount of attrition anyway unless your army is really tiny.

The attrition % of the army can be seen when you select one.

The only place I've really found it to be significant in my planning is on the steppes - my Ming invasion of the Oirat Horde lands didn't go very well - support level 1 provinces and 10,000 strong armies don't mix very well. I like it though - the steppe should be hard to invade.
