it was a ugly princess, or an ugly prince, if your princess has 3 or less hearts then you have a chance of getting the bad wife traits which lower the chance of getting kids

What do you mean, do you marry your princesses to other factions' princes? That way she is no longer part of your family so you won't hear of her kids. If you're marrying other factions' princesses to your own heir then there's a variety of traits that might affect it, like her/him being infertile, ugly and other stuff like that.

if its a successful marraige then he will leave that faction and join yours, if not then she joins the other faction

That's if she manages to seduce him... I though he meant when the "Marry princess to faction heir" option pops up in the diplomacy menu when you're talking to a foreign princess.

If he is Foreign Faction Heir, his marriage to ur Princess is shown on Family Tree, but not their kids )

If you marry your faction heir to a foreign princess, he will have kids as normal.
If you marry YOUR princess to a foreign faction heir (or king) you will see the marriage on YOUR family tree but, even if they DO have kids, you will not see them on your tree, they are not a part of your family.
If you marry YOUR princess to a foreign person by seducing him (meaning he joins your tree), then they have kids as normal, EXCEPT their last names will all be different and screwed up (not the man's last name).
I'm fearful to try and seduce foreign generals because last time I tried they got married, had kids, and when the kids came of age, the family chart messed up and it only displayed two squares; the starting faction leader and his wife, dead. That's it. I tried to wait for a couple more children to come of age but nothing changed. Is it a glitch or does it always happen when you marry your princess to a foreign general, I don't know because I hardly do it?
