I have a 1900 x 1200 resolution and I turn everything on high. I hit apply, play for a while and then when I close out the game my settings are back to default.

Using a mod? If so you need to edit the preferences.cfg manually.

Notepad. Make sure you click apply then Back, or try changing it from the orginial game, some other mods have this problem.
