Default old, old complaint. Fixable ?

Is there any way at all to either prevent foreign troops from entering my lands without permission, or, if they do, to change diplomatic options to demand their immediate removal or, failing that, make it so that entering foreign lands without permission is an automatic declaration of war ?

We've all been there. You're at peace with a faction when, all of a sudden, a full stack of their troops moves right next to your capital, which you've left garrisoned with a minimum of militia. In real life, this would simply never happen; no nation would tolerate this affront to their sovereignty.

Other games, such Civ, react to such transgressions, yet TW games don't even notice (except for perhaps a drop in relationship status). Is there any way to mod this behaviour ?

Sadly, no. Diplomacy options are hardcoded I'm afraid. The only way to make them remove their armies on your lands is by defeating them.

Good point! There should be an option in diplomacy like "remove your forces from our territory"
Also the AI shouldn't do this so often since they should know it's a form of agression towards a sovereign nation. If you move your armies through AI lands it reacts with a drop in relationship. So when they don't like you walking around in their territory, why do they do it in your territory?
When I play Venice or Genua, Sicily always moves its armies through my lands when they are at war with the byzantines or Moors. just ****ing annoyning. The millitary acces treaties don't have any real affect on AI, since they 'll use your territory anyway.

Its build into the RTW engine. Gotta wait for Empire for a diplomacy engine integrated into the AI engine.

I agree, this is really quite a crappy way of emulating real life - especially the military access treaties, which are basically jokes.

I especially hate it when an AI faction just leaves a huge stack next to one of your cities. You never know if it's a precursor to an attack, or the AI just being AI.

I really don't think an automatic declaration of war is a good choice either - the AI factions who I'm allied with do this the most often.
