I read that the vanilla M2TW limits each couple to four children spawned. Can I have more than four children if the earlier offspring die or are wed off to another faction?

I don't believe that they can have more than four even if some of the children are removed by death, desertion or marriage. There is a file called descr_campaign.db where you can set the max amount of children, which is by default 4. Though it might be a hardcoded limit also, haven't tested it myself.

Another quick question if someone can help answer -- do I have to do anything to export the descr_campaign.db altered file/values? Does it have to be a new campaign?
I believe it's possible to get more then 4 children by altering the file. However there still will only appear 4 on your familytree. So if you set it higher you have some guys off the tree but still part of your family (thus they can inherit the throne).
Also, if you have a daughter you can marry her off to one of your in-tree generals or even real family (as opposed to another faction, where that general/heir pops up on your tree). That makes her a 'wife-of' so and so and thus no longer a daughter, then you've opened one spot on the tree. Although I'm not 'sure' that new empty spot can be used to get more children, I'm fairly certain that it can

If he has 4 daughters you can marry them to the sons of another general (within your family tree), thus opening up slots for new children. But then you'll be marrying cousins to eachother.
Also, if you change the file where you can have more children, then he still can have sons, they just won't show up on your family tree. But you are still confined to the ingame max of family generals which is bound the the number of regions you hold.
