"Something" like cheating?
Anyway I don't know a cheat but you can send your spies to locate a settlement somewhere that suffers from the plague (there should be a few at all times) contract the plague with the spy (by entering the city) and use the spy to spread the plague to your enemys' cities by having the spy enter them. Very cheap and old trick, just in case you didn't know it.
If you need to transport the spy by a ship make sure that it's a single ship and don't use it again until it's no longer affected by the plague as any units travelling will be similarly affected.
As far as I know there is no way to start the plague in a settlement - it would be a useful(?) means to control exploding populations.

I do not believe there is a cheat for this. I would just wait until some enemy has a disease and send spies and then give em to your cousin. Just DO NOT attack enemy lands that you just spread a disease to, I did this once with Asia Minor and ended up having to sit my armies in various towns for about 4 years waiting for the disease to pass, while losing about half my men and having to go back and retrain them all.
Forget the Cod this man needs a Sturgeon!
Heh I always welcomed plague in vanilla RTR - they reduce the overgrowth pressure and make my citizens happy

I must admit, it seems odd that during a plague everyone gets happier when lots of people are dying....
Could a script alter this so that a plague causes unhappiness?
And why does a population boom cause happiness?

There is a script for that somewhere around, not 100 % effective but it works.
Because it isn't happiness, it's "Public Order"; people generally don't rebel when dying in droves.I must admit, it seems odd that during a plague everyone gets happier when lots of people are dying....

Yeah, everyone's a bit more concerned with NOT DYING than civil disobedience.
Maybe that's what we need in restless countries! Seed the plague virus!

just take an infected spy and walk him over to your enemies settlement, he will infect the whole city lol.
Although plague does happen within the game it really doesn't break out that often if you've built the correct 'health' buildings.
I know that early in the game the plague will break out in Macedon, no matter what level of 'health' buildings are in the settlement.
When that happens put one of your spys in that settlement and then let them move around infecting the enemy settlements.
If you keep your settlements stocked with several spys and assassins the chance of being infected by the AI is considerably less than if they were not there. So, before you gift the plague to your enemy make sure that your settlements are well protected - otherwise, you may be given the problem again.
The 'health' buildings will also reduce the time that your settlement is affected.
The infection does not just affect the settlement but the agents (diplomats, spies and assassins) within the settlement.
