Remove them. They mostly suffering the disease longer and can reinfect the city again.
Officer to a soldier who refuses to fight: There three types of soldiers who don't have to fight. They are called KIA, MIA and POW and you are not one of them.

I just had a bout of the plague in most of my settlements and you don't get the little skull emblem on the city like you did in RTW which can make it difficuilt to remember which settlemets are infected when your empire is big

@ anvil: well you do have that little rat symbol instead so you can tell where the plague is.
Well i've played the game many times and I know when the black death comes so when it's near I take them out and put them in forts they don't get infected it's strange in this game how the cities dies but the countryside live....must be the fresh air.
Plague can affect characters in the countryside as well as I have lost many a priest to its effects and I always keep them outside. As for moving generals there seems to be as much of a chance of them contracting it as not and by moving them you risk spreading the disease. Sort of damned if you do and damned if you don't.
I don't think your generals would enjoy standing in the wilderness that much. They will get less loyalty and can get some unpleasant traits.
What I did when I saw that plague message was to move all my troops out of settlements with minimum garrison to keep order. Because the the plague hits a few turns later than the message, your troops won be affected, and the plague seems to go away sooner.
