wen i take a city i usually sack it as it is a source of cash. however, wen i capture a city which has a different population to me and will be hostile i usually exermitate.

Cities that are quite friendly or too small there is no need to sack. Just occupy. Cities extremely hostile (we are talking way too hostile) may be candidates for extermination.
But i prefer to sack cities only. It gives a healthy amount of cash, plus eliminates possible riot elements (not as effectively as exterminating though).
I avoid extermination because it raises dread and also make the relations with the opposing faction even worse, minimizing possible ceasefire. Also having more dread is not looked favorably by the other factions too.

Exterminating- You and your army go through the city and murder anybody that opposes you. This will give you the highest public order since anybody that is against your taking the city is dead. This will also render this city useless for many turns as the population drop will be sever. Side effects of this are, decrease in global reputation, and will cause your generals to become dreaded. Can be good if taking a Middle Eastern City if your from Europe (due to the guarentee of low public order)
Sacking- Great way to make a quick buck. Your army goes through and pillages and plunders the city. They will kill anybody in their way. This will kill off some population but not nearly as much as Exterminating. I prefer this to extermination as it makes you more money and will help your public order a little bit because you will have killed some of the opposition.
Occupy- This is what I do most of the time. It leaves the city with the lowest possible public order because you didnt go on a killing spree when you took the town. If you are christian and its a christian city you will most likely be fine since religious order will be maintained. Immediatly build up the public order buildings in this city and then move onto the merchant buildings. Side effects: Generals will become more Chivalrious due to their restraint in slaughtering of the innocent civilians. Also in the long run this will make you much more money as your city will maintain its population and you will be able to build more advanced trade buildings quicker than if you sack or exterminate.
As for your other question the game will go on indefinatly. So play however you want to. Just because you don't "win" the game by the games definition you can still have alot of fun. If your getting bored of the vanilla win conditions make up your own. Maybe a little roleplay is in order. Set a goal and try to accomplish it.

Wether I use occupy/sack/extermination depends on playing style mostly. If I play a catholic faction I prefer occupying to get a good global reputation. If I play the Turks I use sack/extermination mostly (depends on public order), since nobody likes the Turks in my games and no-one wants to become my ally unless I pay huge amounts (even at the beginning of the campaign).
The effect of each action on global reputation are fixed (reputation varies between -1 (despicable) to 1 (immaculute)):
-occupying raises it by 0.05
-sacking lowers it by 0.02
-exterminating lowers it by 0.05
So you could alternate between sacking/occupying and you will eventually end up with a higher global reputation, unless you break lots of alliances and do other bad things.
The three options also have an effect on the recruitments that are possible upon capturing. The effects are pretty straight forward, occupying hardly has an effect, sacking has a medium effect and extermination removes all possible recruitments (except for agents).
