Just hover over the population and the game tells you how many people you need for the next level.

You need 4500 people for upgrading to fortress level, and 9000 for a citadel.

Put a good general in the fortress and the growth rate will increase. Perhaps roads and ports will indirectly stimulate growth by increasing the trade. Does anyone know is such is the case?

You can look at the settlement details panel and it will tell you exactly what is influencing your population growth.

For castles/fortresses/etc, put in a *high chivalry* governor and watch the people flood in.

Plus, as an added bonus, if you are a catholic faction you'll attract religious orders to your region that will allow you to recruit very good knights.

Yes, the trade adds a small growth bonus.
