they'll certainly attack you after a while, but perhaps (fortunately) not as quickly as in vh/vh..
in rome, the H/H-VH/VH settings were essentially cheats for the a.i, giving them alot of money for nothing and putting the player at a disadvantage.
perhaps that's changed for med2, but i still prefer m/m most of the time, too, as it's, as you said, not mayhem, and seems to run alot more slowly and realistically. good if you play on half a year per turn, too.
maybe you should just up the upkeep for your rival nations in data/campaign/imperial campaign/desc_strat.txt ?
then you can have all the goodness of m/m, with the rivals having alot more soldiers and more bravo.

Get a good campaign AI mod like UAI if you haven't done so yet. H/VH has been the perfect mix for me.
