How much can you mod city walls and city-size? I think it would be not only more interesting, but also much more accurate if a city would extend outside the city wall. It theoretically should depend on the size of the population, so if a city is just a few people short of the next wall upgrade, it should extend far beyond the walls.
Also, is it possible to make it so that previous walls remain, even after you upgrade? This would also not only be more accurate (look at any medieval city, like Constantinople), but also provide more difficulty in attacking.

Whenever you look at pictures of medieval cities from the time, you always see houses extending outside the walls. They're not as claustrophobic as inside the walls, but they're certainly more concentrated than the occassional farms you see outside the walls in M2. Maybe you could manipulate the thing that puts those farms there already to put more buildings?

    Just a thought.

Unfortunately, editing the settlement layouts is not possible yet. And even if it were, we don't have skilled individuals who'd know how to anyways.

As far as how settlements look.. expect no surprises or new things - BC settlements = Vanilla.
