1. When you are about to install M2TW you are given a choice where to direct it. There, beneath the sentence "The '[whatever-name-of-the-thing-to-be-installed]' files will be installed in the following directory", the computer has suggested where to put it, e.g. "C:/Program Files/SEGA/Medieval II Total War". You can change that to whatever place on your computer you want. For example, I changed it to "C:/Games/SEGA/Medieval II Total War". When you're happy with the choice, click OK/install.
2. Next you must install the 1.2 patch for M2TW to the same place where M2TW went. So, if you went along with the computer's suggestion in my example, beneath the sentence "The '[patch 1.2-for-M2TW]' files will be installed in the following directory", it must read "C:/Program Files/SEGA/Medieval II Total War" again before you click OK/install here. In my case, it had to read "C:/Games/SEGA/Medieval II Total War".
3. NOW you're ready to install TLR for M2TW 1.2. Do it in exactly the same way as you installed the patch 1.2 for M2TW in the previous step.
After that, through the shortcut "The Long Road" in the start menu, you should be up and running with a really good mod!
PS. For the blood thing, if you want more of it, you probably have to edit the numerical values in the "descr_effect_impacts" file. Check my post "How to edit the costs for having people in the field?" in the Deus Lo Vult forum for further help (this post discuss the blood puffs, too). Adapt the info there to the TLR situation.
Edit: I forgot, there's an optional add-on for TLR which not only gives you the most beautiful skies in the battle mode of all M2TW mods, but also a choice of blood levels when you're about to install it. Install this add-on following the procedure of the second step above.
