Quite embarrassingly enough, I play in Medium/Medium...Although I could say I've some experience in strategy, I still haven't completed a Medium/Medium difficulty campaign with breeze... Plus, I've tried a custom battle(s) on Very Hard and it seemed that my strategic ways couldn't work so I assumed some balancing changes happening...

I play on VH/VH in vanilla, LTC, Ultimate AI, and SS, but I have never played this mod before. I will be starting it in a few days, looks very promising. I love the epic scale of this mod and the massive map.

It's going to be even more different next version. Right now I'm working on the campaign difficulty. Medium will be quite different from Very Hard.
It mostly has to do with the economy and the penalties you get.
Personally, I'll be playing with VH/VH.

VH/VH
I always play with Battle difficulty on VH, since this just effects AI competency- which we always want to be as high as possible.
Campaign difficulty is just AI aggressiveness. Different mods have different aggressive AIs, so sometime you have to tweek it to get the right match of aggressiveness and passiveness.
TRL is best on VH for agressiveness though.
I was under the impression that the player recieved penalties and conversely the AI was granted bonuses at different difficulties.

I tried TLR at VH/VH but I found it to be very easy. Easy compared with DLV mod. Main thing that bothered me while I was playing TLR was access to late era units too soon. Since this mod has 900 turns I figured that I'll get bored very soon so I stoped. Don't get me wrong...this mod has some great features and I'm waiting for future versions.
Oh yeah, almost forgot...tweak merchants...my did not had any real competition. Tweak assasins...they are stupid like those in vanila. Also thing that annoyed me...I was in war with poland and I got besieged by their king alone (o well with 27 of his bodyguards). Funny thing was that in besieged city I had full stack. Can you guess the outcome of battle. Well I thought that polish king had some suicide tendencies...but same thing happend few turns later with his succesor

I can't really recall the year when I got first of them but rest followed quickly.
