It is clearly that there are two layers of armor, just like there is always a leather/woolen armor under plate armor during Middle Age.
The guy who did the first one seems to have thought the power armour was skintight, given how he proportioned it. Then again, he is the same guy who did that really weird feathered Eldar thing.
Technically the so called "power armor" refer the inner suit itself, which is a mechanical suit that help the wearer to perform mechanical tasks that wearer normally cannot do (the mechanical suit itself does not offer much protection however). The main protection device is the outer ceramite plates, which SM cannot wear without a mechanical suit to lift the weight of those plates (hence why it needs a power armor to do so). That is also what modern concept of power armor is - the main protection is the outer plates which require the mechanical suit/power armor to lift up, but the mechanical suit offers very little protection at all (the cables may be able to block some fragements).

Yes they do, there's a lot of worlds that are keep like that I think. It's far more easy to control the population and crush any revolt if the most advanced piece of weaponry they have is a pointy stick.
There's a lot of medieval and agri worlds in 40k. Gist of it is that when the human federation broke apart some worlds regressed to a more primitive state due to not having the resources to maintain the zomg levels of technology, and they remained so in 001M42 (see what I did there Games Workshop?)
