He was referred to as Ser because of his status level besides the position, which was a landless knight.
Janos Slynt was referred to as Lord, because he was a lord.
The actual title for the position is simply 'Commander of the City Watch', but holders are more often referred to by their status in society otherwise.
IE, prince, lord, ser, etc.
Only after he captured Tommen, when Tyrion gave him to a lordship. Anyway, even under LC the title bearer still does not get to be called "lord". It's Ser Barristan Selmy, Lord Commander of the Kingsguard, not Lord Barristan. The exception seems to be the Night's Watch, but, then again, Mormont had already been a lord.
So the AGOT mod for CK2 has finally got its own sub-forum on the CK2 Modification forums. They have some dev journals up about the Essos project they are working on.
The most interesting of all the new features (besides colonization and the ability to reunite the Valyrian Foothold) is that Wildlings can get events where Giant characters will join their ranks.

Sorry to me that doesn't change the fact that rhaegal died and sorry but the Targs aren't the only one with Valyrian blood and characteristics in westeros. However I guess it is up to everyone how they want to rp their characters interpretation of the event. As since the Stark girl was convicted for the banditry in the Reach in the trial by combat. Admitting Rheager's existence seems like you admit you could have been wrong. Just my view of events.
Edit yeah have been gone awhile. still based on what I know at the moment I have no reason to suspect Rheager wasn't the figment of a terrified girl fleeing her homes imagination.
