Are the houses with canon members of the Kingsguard besides the Lannisters (Martell, Hightower, Whent, Dayne, Selmy, and Darry) required to have the member of the Kingsguard as part of the house or is it optional?

Personally I'd prefer it if they were part of the Kingsguard or else they will be NPCs which isn't as fun.
