When my roommate was deleting some saved games on Skyrim he thought the game had frozen and threw a fit and started pounding on the "A" button apparently on the X-Box. Game wasn't frozen, controller was just glitched. He deleted ALL of my saved games and cost me like 15 hours of playing time.
My Roma Surrectum would quite frequently (like every battle) CTD after every battle. So imagine having to fight a battle for an hour, sometimes very important heroic victories, only to get back to your desktop after that.

Shale: My heart does not qualify as shiny. I kill. Frequently, and not without pleasure.
Leliana: You had a difficult life. Deep down, at the center of your being, you are a good person. I believe that.
Shale: Even though I have never demonstrated this aspect? How peculiar.
Leliana: You aren't all stone, Shale. There is a person inside of you.
Shale: If so, it is because I ate it.
For me: I had this rivalry with a friend of mine in Super Smash Bros 1. I was Link and he was Carby, and in this all-or-nothing-who-is-best game we were both down to one life and at the end I stood at the edge and saw him slowly fall to his death on the Fox cornelia map.
Inn comes a walking bomb and blows me away. I died one second before him and lost the match.
That was 12 years ago. He still gloats over that occasionally.
This is a recent one, spending about 6 hours straight on a Dont starve character, only to get distracted and not get a fire up in time when it got dark (DEAD).
This...Mass Effect 3's ending for me. Just came totally out of the blue and blew me right out of the water with how it was. The DLC didn't really fix it imho, either.
Then, as throngs of his enemies bore down upon him and one of his followers said, "They are making at thee, O King," "Who else, pray," said Antigonus, "should be their mark? But Demetrius will come to my aid." This was his hope to the last, and to the last he kept watching eagerly for his son; then a whole cloud of javelins were let fly at him and he fell.

Every single ****ing checkpoint save in Max Payne 3
More like every single unskippable cut scene.
(Yes, I know, the cutscence is loading the level but jesus....)

well only the last final 10 minutes were that bad, the rest was superior to the previous 2 games ^^
but using your words i would say that Aliens Colonial Marines "nearly killed me from disappointment" but i survived but had nightmares on the following days, because how badly this game ruinned the whole "Aliens" franchise...
Common sense removed due being Disruptive.
I think pretty much any aliens FPS game designed with console in mind will probably suck. With AVP (the new one) it was designed with consoles in mind first and foremost, the aliens movements became scripted set pieces and they moved at an incredibly slow pace. With AAA mainstream budget as well they over-script everything and want to make the game as accessible as possible.
The great thing about the original AVP was the aliens didn't predictably spawn in the same location, they never stopped spawning and they moved at a fast pace (designed with mouse and keyboard in mind) and could pretty much insta-kill you. It pretty much perfected bringing them to a video game imo.
Frustration is something we usually encounter in tough games but pain? That is only when some human player rips us apart. I still remember my bitter defeat when i lost several matches with my cousin playing tournament fighters. I used to think that i was unbeatable in it until i met my cousin. Lost several bets & ultimately broke my video game in anger. Even after so many years he still gloats about that incident.
This is the game i'm talking about:
