I have no idea how Hatakeyama (Green, except the one in the center, that's Kisho) managed to take all these territories.

I've seen patchwork territories show up throughout my campaigns. I've seen a few naval invasions and the AI is more than willing to cross an allied province or two to conquer a province. While they start consolidating into superpowers about half way through. Until then its pretty free flowing.

Technically speaking, they didn't actually hold those areas in conjunction with each other. They were seperate entities, with what I suppose would have been cordial relations (though considering two of the three territories was rife with civil strife, I'm not certain they could have been that friendly).
I suppose one should look at them like the Uesugi. Ingame they have been split up into the Uesugi proper and the Ogigayatsu, which was merely a branch of the Uesugi, but one that wasn't really very friendly at all with the rest. And since Kenshin belonged to the other branch (or rather joined), it became the Uesugi clan ingame.
Now the logical question is, why the destinction between the Uesugi branches but not the Hatakayama?
Well, the Uesugi is a playable faction, and having the Ogigayatsu territories would be a considerable advantage. Meanwhile an AI faction having three very widely spaced provinces isn't really going to topple many powerschemes by other factions. Basically there was little gameplay reason to split them up, on top of the fact that I haven't seen any branch names, like the Ogigayatsu for the Uesugi. CA wouldn't be forgiven had they invented some clan names.
