That is exactly where they belong. There are many medieval worlds in the Imperium.
The whole problem with the world is that fools and fanatics are so certain of themselves, but wiser people are full of doubts.

Well it's an official comic about Tank Red of the Black Templars.

On that matter Space Marines tend to recruit from Hive Worlds, Medieval, and Feral as they produce hardy troops.

I think its ridiculous to suggest that their technology would revert to medieval status just because they lost contact with another planet or, didn't have enough resources. "Not enough power!!! Turn off the crazy machine." Simple

Technology like, the light bulb - created after the middle age - would be almost impossible to erase.

Real Humans have been around for more than 2000 years, and look at what we have accomplished.

If you want a compromise, then the acceptable answer is, people in 40k must be really, really stupid.

Ultimately it's a fictional universe with space elves, space orcs, and psychically powered human supersoldiers that worship an immortal emperor of the Milky Way. Why, with all that said, is the existence of medieval colony worlds the thing that sets you off?

Well IZob, in the garden of the Emperor there's something of everything. Generally the focus of some things is on medieval worlds because for example, SM rather recruit from there than from pampered "civilized worlds". Don't quote me on this but I recall reading something about most worlds on the Imperium actually being kind of like ours but with more tech and this uncivilized worlds actually being protected by special "laws", kind of like forest reservations of sorts.

Also, there are numerous ways to a society go back like that. Imagine a mix of being on a planet dependent of imports for energy, without oil or coal and suddenly cut off by an alien invasion. Let's say they fight off the invasion but are cut off for literally thousands of years, most of their infrastructure destroyed and now trow in a little old fashioned sects and cults and have yourself a merry little Christmas.

That's actually intentional. The artist intentionally mixed up characters from both.

Well, actually they are in a weird position among the Loyalist chapters .. their passion for blood is very close to Khorne ideology, not to mention the Black Rage, The Red Thirst ..

Just a quote by somone very close to them:

    "Roboute Guilliman.

    Guilliman the tactician.
    Guilliman the warrior.
History will remember the primarch of the Ultramarines by many titles. To us of the Blood he will have only one – Guilliman the butcher.
With law and edict, Guilliman ripped the heart from the Legions.
Even his own sons were not spared his treachery.
Where Horus had sought to use a hammer blow, Guilliman used a duellist's blade. In the end, the result was the same.

Brother became cousin. Cousin became exile.
Guilliman's new chapter, his new beginning, was a sundering to all that had come before.
Stripped of honour and tithe, of history and deed, we were all of us undone.
Bastards of war and victory, we were Angels no more."

    - Flesh Tearers Chapter Master Nassir Amit
They are midway between Heresy and Loyalty, between Honor and Blood thirst, between the Golden Throne and the Red Thirst, anyway all this looks very, very British!
(.. and if you consider that actually also Cameron is midway between Black Rage and .. European Union .. )
