Has anyone actually had their character die at the end of Mass Effect 2? I have heard it can happen but I have yet to meet anyone that it has happened to.

It has not happened to me personally but I do know that you actually have to plan on killing Shepard. Accidentally getting him/her killed is extremely unlikely if you play the game in a "normal" way.
There are some vids of it happening on YouTube. I think he dies if you don't have any upgrades to the Normandy or something, otherwise I can't imagine how.
Simply having all members of the team not loyal. then you die. because they all die.

Non-loyal team members can still survive though.
Easily might I add.

it's not that hard to figure out who's better at what job. they tell you half the game how jack was built as the ultimate biotic. damn right she can do the job right.
garrus already had a team and bombed half of Omega, they were loyal to him so he's a def candidate to lead your second team.
and when it comes to tech for the tunnels, it's really between tali and legion. I always pick Legion... he's a damn robot who can hack things by just hovering his hands over a terminal. Plus he has a sniper rifle and he's made of metal.

Well I knew Samara or Jack were the best choices, but I had sex with Miranda and ever since then I had really been marginalizing her efforts during the suicide mission so I let her put up the Biotic field since she said she could do it lol, damn her she cost me Thane!!! Of course Jack wasn't loyal either so I couldn't pick her to do and Samara I was using in my party. I used Tali to go through the vents. And Garrus led my teams, as usual. In any case, Thane was one of my favorites so it really sucked having him get wasted =( .
Btw, what happens if you play again the 2nd time and import you ME2 character over again. You get all your stats and items etc, but how does it affect the story? You can go through and do everything different and have two separate exports for ME3 or what?

new game + is much more difficult than the first time through.
And I know you're some Tough Marine Guy™ who always plays a soldier, but you should try a sentinel, it's quite fun and really powerful.

I hate how we get to the character customization screen after having to sit through the whole Normandy attack.
I also hate how we don't know the details of our imported ME1 game until we load it up.
Well, a direct import from ME1 will recieve a paragon/renegade bonus for their first ME2 playthrough. Funny thing is though, if you take that character and start a second ME2 playthrough with them, they won't start round 2 with that paragon/renegade bonus from the import.
