@Raglan - Thanks for asking!
AFAIK it is. Waiting on review and okay on survey questions from Augustus. He initiated the process, figured it could be him to "hit the gas" on it.
(The effort is in need of some "executive sponsorship activity" ... .. "brass" needs to pull the trigger and we need to re-engage the ones who've offered to help with distribution.)
Then we can take it to the next level.
Intro Text - We need some copy for the introduction to the survey, general stuff - why we are doing it, etc.
Verification still seems to be an "open issue" - but we can use email addy (not very reliable) and TWC names ...
Decision on platform - if we want to change from what we have - like a place to make custom skins and stuff, then discussion needs to be final on that. Or we go with what we have...not a problem to me.
Implementation Troops - once those things are out of the way, then certainly we can start the announcement, and communication part - there are others who can carry the torch from there.
DD and I can work the results end of things - process the data into numbers and perhaps even load em into a database for further processing by someone with more time/experience than we have.

Which post has the most up-to-date list of questions? The OP can be edited with it if you like.
Regarding platform, unless I can stab my way through re-implementing this on a PHP page, which seems unlikely, we're probably stuck with the Google option. I'll look into it though.

I modified your top post in the Questions Thread so that it points to the draft survey with the questions there. (hope that was okay)
Also - re-read the PM i sent you about platform choices. I think doing PHP level work for this would be insane. If we absolutely want to change platforms we can - but let's use a free one. Copying the questions shouldn't be too difficult. (it's coming up with them that's difficult!)
