Is it possible to have a warning type that is recorded in the profile, but is invisible to the member (and visible only to moderation staff)? No automatic PM sent along with it of course.
Permissions to see infractions would be handled on a usergroup by usergroup basis, rather than an infraction by infraction basis. It would need to be hacked.
Yes, it would need a hack. It might be possible to repurpose user notes for this, but those are less convenient – displayed in a totally separate place.

Notes are often utilized as notes to other moderators. When a moderator sends you a PM for instance saying "stop doing X" that's well and good for that moderator, but other moderators can't know they spoke with you about it. They could leave a note saying "User has been informed to stop doing X by PM".
That's just one example, I don't know why it's desired in this case.
Oh, you don't need a hack for that. Look here. On the 'About Me' page you'll see at the bottom in the "Name-Calling" field that I called him a Doodoohead. Only people with the permissions to see private custom fields can see it, and users can't edit it themselves. Needs to be added through the Admin CP though on the user page, but we all know it's you corrupt admin lot that want to use it anyways.
