Every time I start the Regions & Provinces Mod from its .exe on my desktop I need to change the video settings to max.. seems like all settings video, sound etc resets to its default every time I exit the mod.
How come? Can it be fixed?

How come? The programmer didn't give the game the brains to update the files on the fly, hence reset to default. You can set them manually in the RegionModBM.cfg.
