So hard, so hard of a choice. Here's my top three.
Darth Vader- Don't argue, sure its cliched, but its cliched because he is such a hardcore villian. The suit, the force power, the voice, everything about him just cries evil. How many of us have honestly been him for Halloween?
Hannibal Lector- Frightening. He's the guy who seems so cultured and refined, but he eats people and is insane. Anthony Hopkins' performance as him haunts your dreams. The mind games he plays with people are intriguing as well.
John Doe, from Seven- I did not know Kevin Spacey could be like that. So calm, so calculating, so deliciously evil. I mean, HE WAS K-PAX FOR GOD'S SAKE!!!

Kevin Spacey also made a great role in The Usual Suspects as Keyser Soze, or whoever. I had no idea he was that great of an actor.
Hannibal Lecter, truly good at the role of a sophisticated cannibal.
Of course, I think Vader takes the prize, and the Emperor Palpatine, they're both really villainous.

Mark from The Headmaster Chronicles Volume III. He's suave, possesses a veritable rapertoire of witty catchphrases, has expert skill with his twin pistols, gets his eye scratched out by seagulls, takes on Airforce I with his harrier jet, lures all 6 billion people in the world to do battle with him on Antartica, takes on his dad (and wins) dressed in a full metal suit of armour, then fights the final battle in the ice wearing just combats and a hell of a lot of bullets... :w00t

I'd say the three greatest movie villains, in no particular order are:
1. Darth Vader
2. the alien from Predator
3. Johnny Ringo (I think - whoever the guy Doc Holliday went up against) in Tombstone
The best villain of all time (in any visual media) is Al Swearengen (sp?) from Deadwood.
I think the quality that makes a villain great is an ability to communicate, verbally or non-verbally, why they are a villain. You can understand, and maybe agree with, what drives them to their villainous behavior. If this quality isn't present, they become mediocre and sometimes stereotypical charicatures.
